  • Once you login and navigate to the form. It’s easy to complete as an electronic form is available to populate with information, attach applicable documents and submit directly to the ADC CPA team.

  • The information is stored in a software that follows ISO/IEC 27001:2022 specification for an information security management system (ISMS). To ensure the system is working, there is a Service Organization Controls Audit (SOC 2 Type 2) that is completed to assess the protection of practice and patient confidentiality.

  • If there is an error in the submitted form. It's best to complete a new form with a note in the comment field to indicate a prior form was completed with an error. Please note that payroll forms are time sensitive, so please submit a correction form as soon as possible.

  • Please email info@adccpa.ca with details pertaining to the login error message and your username. We can look into the login issue and respond back within 1-2 business days.
